Description:To be fully engaged in providing Quality/No Harm, Customer Experience, and Stewardship by providing administrative support and assistance to the position of report and by performing a wide variety of patient care and management activities as directed by the attending physician, Program Medical Director and Program Director. The Behavioral Health Therapist is responsible for a patient caseload and will provide individual, family and group therapies as ordered by the physician. The Therapist will conduct screenings and psychosocial assessments as directed and will act as case manager for those cases assigned. Case management responsibilities include maintaining contact with the referral source, family and other caregivers and service providers, as appropriate, and coordinating discharge planning activities with the treatment team and utilization review as necessary for admission and continued stay. The Licensed Behavioral Health Therapist collaborates with the treatment team in formulating the Master Treatment Plan and associated updates. The Licensed Therapist participates in the referral development process and is an active member of the Referral Development CommitteeDocuments all clinical and case management services and activities provided according to CMS and TJC requirements.Participates in review and revision of the Master Treatment Plan in collaboration with the treatment team.Complies with all administrative, clinical and human resources policies and procedures and with all relevant standards as promulgated by CMS, TJC and state and federal regulatory agencies.Adheres to work practices consistent with Health First Palm Bay Hospital missions, Medical Staff Bylaws, and administrative, departmental and human resources policies and procedures; maintains the highest level of ethics in all areas of program activity.Participates in activities relating to the Program's quality improvement plan, assessing the quality of patient care by collecting data using established indicators; participates in utilization review and risk management/safety plans as directed.Provides pre-admission screenings and psychosocial assessments and develops the Master Treatment Plan with input from the treatment team.Provides therapeutic/crisis intervention counseling with patients and families as needed.Assess and document in MIDAS and electronic medical record patient/family needs and collaborates with physicians and staff to identify possible psychosocial related discharge needs.In a crisis situation, where safety of the patient or others is in immediate jeopardy, the licensed therapists can Baker Act a patient.Completes mandatory reporting processes.Facilitates referrals for post-discharge psychosocial and therapeutic needsAssures compliance with regulatory standards of care at both the state and federal levels.Assists with patient meals as necessaryOther duties as described by Supervisor/Manager/Director.Provides individual, family and a variety of group therapies (e.g., cognitive, process, psycho-educational) as ordered by the attending physician.As case manager, coordinates discharge and aftercare planning with the Program Nurse and other treatment team members and communicates with family, caregiver and residence regarding changes in treatment.Actively supports the mission, philosophy and goals of Health First Palm Bay Hospital.Assists in the admission/intake process as directed, including completing required admission. paperwork and meeting with patient and family on admission to provide orientation.Participates in Referral Development Team activities which include, but are not limited to, visiting facilities and programs representing the Center's mission and program, providing relevant seminars and in-services to both providers and consumers, and actively networking within professional and personal spheres of influenceQualification:Master's degree in social work, psychology, counseling or a comparable behavioral healthcare degree.Must hold current license as a LCSW, LMHC, or LMFT in the State of Florida and be in good standing to practice respective discipline independentlyMust hold current AHA BLS Healthcare Provider Completion Card prior to start date and maintain.Personal computer knowledge necessary with proficiency in Microsoft Windows, and Microsoft Office applications.Good working knowledge of various type of office equipment (fax, copy machine, etc.).Experience in working in a behavioral healthcare facility with geriatric experience preferred.ServSafe Food Handler Certification preferred
