HCA Healthcare is seeking a highly skilled and experienced RN Case Manager Lead to join our dedicated team. This pivotal role involves leading a team of case managers in delivering exceptional patient care, optimizing patient outcomes, and ensuring seamless coordination of healthcare services. Lead and supervise a team of RN case managers, ensuring effective patient management across various units. Develop and implement patient care plans in collaboration with healthcare team members. Coordinate and evaluate all aspects of patient care, including resource allocation and discharge planning. Maintain compliance with federal, state, and local regulations, and organizational standards. Assess ongoing training needs and provide mentorship to ensure professional growth and high-quality care. Implement quality improvement initiatives to enhance patient outcomes and service efficiency. Strong leadership and organizational skills Excellent communication and interpersonal skills Ability to analyze complex information and make informed decisions Proficient in healthcare management systems and software Experience in patient care planning and case management Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Active Registered Nurse (RN) license Certified Case Manager (CCM) preferred BLS/ACLS certification
