HonorVet Technologies is seeking a travel Home Health Physical Therapist for a travel job in Melbourne, Florida. Job Description & Requirements - Specialty: Physical Therapist - Discipline: Therapy - Start Date: ASAP - Duration: 13 weeks - 40 hours per week - Shift: 8 hours - Employment Type: Travel Title: Physical Therapist – Home Health Location: Melbourne, FL Duration: 13 weeks Shift: Day shift, 5x8 hours (08:00 – 17:00) Job Responsibilities: - Provide physical therapy services to adult and geriatric patients in a home health setting - Perform comprehensive patient evaluations, assessments, and reassessments - Develop, implement, and update individualized plans of care - Establish and document short-term and long-term goals - Complete OASIS documentation accurately and timely - Provide discharge planning and patient/caregiver education - Develop and instruct patients on home exercise programs - Address safety, fall prevention, and injury prevention in the home environment - Manage time effectively to meet daily visit expectations (approximately 7 visits per day) - Document all care and progress using the Epic electronic medical record system - Collaborate as needed to ensure continuity and quality of care Required Skills & Qualifications: - Minimum of 1 year of physical therapy experience - Home health experience required - Active Florida Physical Therapist state license (must be in hand at submission) - Current BLS certification - Epic EMR experience required - Prior charge experience required - Experience with adult home health patient populations - Strong documentation skills, including OASIS - Ability to work weekends as required - Two professional references from supervisory or leadership roles within the last year Preferred Skills & Experience: - Strong time management and organizational skills - Knowledge of medications and patient safety practices - Experience educating patients and caregivers in the home setting - Ability to work independently while meeting productivity and quality standards HonorVet Technologies Job ID 31623287.
