Job description:
Manages each patient's hospital stay by working with hospital
staff, physicians, and insurers to provide optimal care within the
LOS and resource utilization expectations. Assists the patient and
Family in identification of post hospital needs and assists the
patient and family in obtaining the services. Assists the patient
and family in all phases of transfer to either another acute care
facility or to transition to care within the health care
continuum.
